Confessions of a Dermatologist

This derm’s confession?  I am a control freak, especially when it comes to my appearance. Because even good stress can take a toll on one’s skin, every year I have the five weeks between Thanksgiving and New Year’s to deal with my misbehaving complexion. Going to cocktail parties, battling the crowds while shopping for last minute gifts, packing patients into my schedule for their  holiday “fix,” all while I’m sleep deprived and sustained on a diet of sugary carb-laden fatty foods, does not wear well!  A flare-up of sensitive skin, breakouts and under-eye dark circles comprise my personal battlefield.

Fortunately, as a dermatologist, I have knowledge, experience and skills on my side to whip my skin into picture perfect shape, making sure I look as fresh and rested at my New Year’s Eve party as I did at Thanksgiving dinner.  Here are my top tips to hide stress and prevent it doing a number on my skin.

First, I try to meditate at least 10-15 minutes a day. Meditation helps to control cortisol, the hormone which is released in response to the “fight or flight” reaction.  The skin signs and symptoms of cortisol excess include an increase in TEWL (transepidermal water loss) which causes skin to become dry and itchy.  Also, cortisol produces a spike in sebaceous gland output contributing to breakouts which can really ruin your family photos. While I don’t have the natural temperament to meditate and most days don’t feel like I have the time, fitting this practice into my routine really helps take the edge off holiday stress.

Second, I always address the visible signs of stress-induced inflammation, like redness and irritation. My favorite methods of masking these flare-ups is with the powerful combination of SOOTHE Sensitive Skin Treatment and ENHANCEMENTS Mineral Peptides. All stress is “bad”

Very few people would classify positive life changes, such as a job promotion, new baby or a wedding as stressful. However, even these exciting life events can become sources of stress in the form of added responsibility and pressure. Unfortunately, whether good or bad, stress shows up the same unwanted way – on your face. So take control of the stress in your life, both good and bad, and relax. Here are some methods that work for us:

  • Find time each day for breathing exercises. Inhale to a slow count of 10, then exhale to an equally slow count of 10. Practice this daily and then, when a stressful situation arises, breathe.
  • Meditate or practice self-hypnosis.
  • Exercise, go for a walk or practice yoga.
  • Stop worrying. If there is a problem, make sure you have all the facts, make a plan of attack and act on the plan immediately.
  • Soak in a hot tub – but not too hot.
  • Get a massage.
  • Take a vacation.
  • Get a full night’s sleep.
  • If your levels of stress seem overwhelming, seek help through a therapist.

Kindergarten and Professionalism

Remember that old poster "Everything I Really Needed To Know I Learned In Kindergarten?" here's the list if you don't remember:

    * Share everything.
    * Play fair.
    * Don't hit people.
    * Put things back where you found them.
    * Clean up your own mess.
    * Don't take things that aren't yours.
    * Say you're sorry when you hurt somebody.
    * Wash your hands before you eat.
    * Flush.
    * Warm cookies and cold milk are good for you.
    * Live a balanced life - learn some and think some and draw and paint and sing and dance and play and work every day some.
    * Take a nap every afternoon.
    * When you go out in the world, watch out for traffic, hold hands and stick together.
    * Be aware of wonder. Remember the little seed in the Styrofoam cup: the roots go down and the plant goes up and nobody really knows how or why, but we are all like that.
    * Goldfish and hamsters and white mice and even the little seed in the Styrofoam cup - they all die. So do we.
    * And then remember the Dick-and-Jane books and the first word you learned - the biggest word of all - LOOK.

These principles can also relate to our business, too.  And, I believe that you will reap what you sow.  So, sow well:)

1. Share everything - Links, content, keywords, pictures etc...the more content you share the better your rankings.

2. Play fair - Google will find out if you don't and you will be punished!

3. Don't Hit People - Always good advice.

4. Don't take things that aren't yours - This includes plagiarizing articles, sales copy, or any other content. Videos and and photos too! Duplicating content doesn't help it hurts!

5. Flush - this is just always good advice too...

6. Live a balanced life - learn some and think some and draw and paint and sing and dance and play and work every day some - You'll never know where great ideas come from. Relax your mind, enjoy your life and take time to be quiet and just ponder...great ideas come to those who are quiet enough to hear them!

7. When you go out in the world, watch out for traffic, hold hands and stick together - Watch out for "traffic." - Priceless. Watch where your traffic is coming from so you can go out and get more of it. Watch where others are getting their traffic so you can get some from the same place. Link together and everybody wins!

8. Be aware of wonder. Remember the little seed in the Styrofoam cup: the roots go down and the plant goes up and nobody really knows how or why, but we are all like that - Remember we are still in the growing process. You start small. Plants seeds everyday, water them with gentle reminders and sunshine:) and watch your teams grow!

Bree, a graduate student in psychology and longtime acne patient, presented with a flair in her condition. This event is a common one seen in students undergoing final exams just prior to the winter break. The fact that Bree’s acne had severely worsened under pressure is no surprise. Research from Stanford University’s School of Medicine shows that there may be a strong correlation between exam-related stress and acne severity in college students. A volunteer sample of students with varying degrees of acne was assessed during both non-examination and examination periods. The result was that as students approached exams and experienced a higher level of stress, they also experienced more severe acne. But unfortunately, acne flare-ups don’t stop once you’ve received your diploma. Acne can be a lifelong condition and flare-ups can be brought on by any of life’s stresses.

The best way to prevent acne is to treat the full face daily with an acne-focused regimen containing either sulfur or benzoyl peroxide to address breakouts, and hydroquinone to address post-acne dark marks. If you’re coming into typically high stress circumstances, double up and apply your regimen twice daily. People with really persistent acne may need to see a dermatologist for more intensive treatment options. In Bree’s case, I recommended an over-the-counter acne regimen twice daily for a month before resorting to a prescription alternative. But daily preventative treatment the month before her finals would have most likely saved her the trip. Because when it comes to acne, prevention is key.

Don’t Stress … Tips for a Beautiful Holiday Season

Black Friday has come and gone and the holiday season is in full swing ... and so is seasonal stress.   Greeting cards, wish lists, holiday menus, party details, traffic. parking spaces, sold-out electronics, lines at the toy store, lines at the post office, lines everywhere, air travel  … phew … even compiling this list is causing a surge in cortisol levels. While it might be “the most wonderful time of the year,” holiday season is often accompanied by a super-sized serving of stress.

Mounting stress needs an outlet or the first place it may show is on your face. Your body responds to stress with an increase in the stress hormone, cortisol.  Cortisol is part of the natural fight or flight response that enables self-preservation.  But when you’re all stressed out with no place to go (like waiting in line at Target), cortisol can wreak havoc with virtually every organ in your body and that shows on your most visible organ, your skin.  The signs are obvious: breakouts, an ashen or sallow complexion, the flushing of rosacea, dark under-eye circles, dull and limp hair, and more pronounced lines and wrinkles. And what’s more, the effects of stress on skin are cumulative so even small stressful events can show on your face.

While stress is a part of everyday life, there are ways to manage it and control how it affects your skin, so you can look great at the party even if you stressed over planning it or getting there.  This month on Derm RF, we’ll give you tips to manage stress from the inside out so you can look your best this holiday season and well into the new year.
